Every Child Deserves a Medical Home
In a medical home, a pediatric primary care provider works in partnership with the family/patient to ensure that all of the medical and non-medical needs of the patient are met. Through this partnership, the pediatric primary care provider can help the family/patient access and coordinate specialty care, educational services, out-of-home care, family support, and other public and private community services that are important to the overall health of the child/youth and family. Primary care providers’ play a crucial role in managing the patient’s chronic health condition, advising patient/families about testing and treatment options, and providing education to parents and patients about their condition.
Shriners Hospitals for Children has made communication and co-management with their patient’s primary care provider (i.e. medical home) a top priority. Shriners Hospitals for Children is committed to providing timely communication regarding their patients’ admission to the hospital, treatment plan, and follow-up, in addition to educational materials on common follow-up procedures.
Since 1996, the Shriners Hospitals for Children and the American Academy of Pediatrics’ (AAP) National Center of Medical Home Initiatives have partnered to promote comprehensive, coordinated health care within a medical home for all children, especially those with special needs.
