The specialist’s role in patient care:
- Collaborate with primary care providers and services in the patient’s hometown to provide integrated care for the child’s condition
- Provide parent and patient education about the condition, testing and treatment options in collaboration with primary care providers
- Provide diagnostic services and treatment related to the child’s condition being managed at Shriners Hospitals for Children - Chicago
- Provide information to primary care providers of findings and treatment and follow-up plan
The primary care provider’s role in patient care:
- Manage patient’s chronic condition holistically
- Provide of primary care
- Coomunicate and coordinate community services with Shriners Hospitals
- Provide patient education abut condition, testing and treatment options
In addition, providing medical information on your patient to us is very helpful to their treatment at Shriners Hospitals. Please send us information on:
- Clinical concerns, including any concerns relayed from the family
- Any specific questions
- Medications
- Allergies
- Family medical history
- Medical history (in general and in relation to the problem)
- Surgical history
- Any supporting info (lab/x-ray results, growth charts)
- Best way to contact you (phone number, fax, times/days , etc.)
